Common use of Personal Services Coordinator Clause in Contracts

Personal Services Coordinator. (PSC) means an individual who will be part of a multi- disciplinary team that will provide community based Mental Health Services to adults that are struggling with persistent and severe mental illness as well as homelessness, rehabilitation and recovery principles. The PSC is responsible for clinical care and case management of assigned Client and families in a community, home, or program setting. This includes assisting Clients with mental health, housing, vocational and educational needs. The position is also responsible for administrative and clinical documentation as well as participating in trainings and team meetings. The PSC shall be active in supporting and implementing the program’s philosophy and its individualized, strength-based, culturally/linguistically competent and Client-centered approach.
