Common use of PERSONS TO BE SERVED Clause in Contracts

PERSONS TO BE SERVED. The target population for the Non-Emergency Transportation Services program is adults, residing in COUNTY, eighteen (18) years of age or older, who have been diagnosed with a serious mental illness, as well as adults with substance use disorders, and require non-emergency transportation assistance to essential behavioral health and/or medical appointments, or behavioral health supportive services that support goals identified in their treatment plans.

PERSONS TO BE SERVED. The target population for the Nonnon-Emergency emergency Transportation 13 Services program is adults, residing in COUNTY, eighteen (18) years of age or older, who have been 14 diagnosed with a serious mental illness, as well as adults with substance use disordersillness and who may have a co-occurring disorder, and require non-non- 15 emergency transportation assistance to essential behavioral health and/or medical appointments, or 16 behavioral health supportive services that support goals identified in their treatment plans.

The target population for the Nonnon-Emergency Transportation emergency 23 transportationTransportation Services program is adults, residing in COUNTY, eighteen (18) years of age or older, who have been diagnosed with a serious mental illness, as well as adults 24 with substance use disorders, and require non-emergency transportation assistance to essential behavioral health and/or medical appointments, or behavioral health supportive services that 25 support goals identified in their treatment plans.

The target population for the Non-Emergency Transportation 36 Services program is adults, residing in COUNTY, eighteen (18) adults over 18 years of age or older, who have been diagnosed with a serious mental illness, as well as adults with substance use disorders, illness 37 and who may have a co-occurring disorder and require non-emergency transportation assistance to 1 essential behavioral health and/or medical appointments, or behavioral health supportive services services, that 2 support goals identified in their treatment plans..
