Place of performance of the services. 1. The Service Provider undertakes to perform Medical Services as described in Annex 1 to the Agreement, in POLMED Medical Centers and in outpatient clinics of POLMED Medical Partners located in the territory of the Republic of Poland. The list of the Service Provider's facilities is available on the Provider's website xxx.xxxxxx.xx and is updated there.
