Pre-Treatment Estimate. A Provider may file a Claim Form before treatment, showing the services to be provided to an Enrollee. Delta Dental will estimate the amount of Benefits payable under this Contract for the listed services. Benefits will be processed according to the terms of this Contract when the treatment is performed. Pre- Treatment Estimates are valid for 365 days unless other services are received after the date of the Pre- Treatment Estimate, or until an earlier occurrence of any one of the following events:  the date this Contract terminates;  the date Benefits under this Contract are amended if services in the Pre-Treatment Estimate are part of the amendment;  the date the Enrollee’s coverage ends; or  the date the Provider’s agreement with Delta Dental ends.

Pre-Treatment Estimate is an estimate of the allowable Benefits under this dental plan for the services proposed, assuming the person is an eligible Enrollee.
Pre-Treatment Estimate. Before beginning a course of treatment, a description of that course of treatment may be submitted to Delta Dental. Delta Dental will provide an estimate of the benefits for the planned course of treatment. Delta Dental does not require pre-treatment estimates. However, Delta Dental offers this Service in order to allow for treatment planning.
