Procedures and Guidelines. a. The Educator Collaborative Assessment Program identifies the Utah Effective Teaching Standards and continuum of practices as a description of effective performance. b. Performance assessment is to be completed by the principal, the principal’s designee or the teacher’s immediate supervisor. c. Classroom observations are required annually for all teachers. As soon as possible, but no more than five (5) working days after an observation, the details of the observation shall be written and discussed with the teacher. After discussion and revision of the observation report, a copy of the report shall be given to the teacher. d. A written response from the educator to all or any part of the assessment report may be attached. e. The Collaborative Intervention Plan shall be collaboratively developed by the administrator and teacher. The components of the Collaborative Intervention Plan shall include: • Statement of specific concern/issue: reference the standard(s) and indicator(s) • Evidence/reason for the concern(s) • Expected outcome(s) of the plan • Time Frame of the plan • Evidence to meet the expected outcome(s)
