Procedures for Withdrawal. a. A member requesting days from the Sick Leave Bank shall complete the application form with supporting evidence from the member’s physician, and forward same to the Sick Leave Bank Committee. The Sick Leave Bank Committee shall have the responsibility to either grant, or deny the withdrawal request. If the member is incapacitated and cannot make application for withdrawal, the Committee may make an application on the behalf of an incapacitated member and may grant the withdrawal on his/her behalf.
