PROFESSIONAL LICENSURE/CERTIFICATION. The Treasurer shall furnish, append hereto, and maintain throughout the term of this Contract a valid license to act as a Treasurer of the Pickerington Local School District issued under Section 3301.074 of the Ohio Revised Code.

PROFESSIONAL LICENSURE/CERTIFICATION. The Superintendent shall maintain and furnish to the Board evidence that he is maintaining (throughout the life of this Contract) a valid and appropriate certificate to act as superintendent of schools in this District in accordance with the laws of the State of Ohio. In addition, Superintendent shall maintain, throughout the life of this Contract, valid credentials to serve as an evaluator pursuant to the Ohio Principal Evaluation System (OPES).
PROFESSIONAL LICENSURE/CERTIFICATION. If Provider is an individual, Provider shall maintain, at all times, an unqualified and unconditional license or certificate in the state in which he/she shall practice, as applicable to his/her profession. Provider shall submit evidence of such licensure or certification to PCN upon request.
PROFESSIONAL LICENSURE/CERTIFICATION. Contractor shall maintain all necessary licenses and certifications related to medical services being provided hereunder. Contractor shall provide Director documentation that the provider’s license is in good standing with the licensing entity. In the event licensure or certification expires, is revoked, suspended, probated, or is canceled, Contractor shall inform County of such event within five (5) working days.

  • Drug-Free Workplace Certification As required by Executive Order No. 90-5 dated April 12, 1990, issued by the Governor of Indiana, the Contractor hereby covenants and agrees to make a good faith effort to provide and maintain a drug-free workplace. The Contractor will give written notice to the State within ten (10) days after receiving actual notice that the Contractor, or an employee of the Contractor in the State of Indiana, has been convicted of a criminal drug violation occurring in the workplace. False certification or violation of this certification may result in sanctions including, but not limited to, suspension of contract payments, termination of this Contract and/or debarment of contracting opportunities with the State for up to three (3) years. In addition to the provisions of the above paragraph, if the total amount set forth in this Contract is in excess of $25,000.00, the Contractor certifies and agrees that it will provide a drug-free workplace by:
