Common use of Provider Business Changes Clause in Contracts

Provider Business Changes. Notify BCBSM within thirty (30) days of changes in Provider's business including changes in ownership, name, tax identification number, National Provider Identifier, location, phone number, business structure, range of services offered and specialty. Prior notice of such changes does not guarantee continued participation under this Agreement;
