Provider Number Terminations by DHS/OHA. ‌ In the event of the termination of a HCW’s/PSW’s provider number, the HCW/PSW will be notified pursuant to DHS/OHA OARs. The HCW/PSW will be unavailable for referral in the Registry and is not subject to the grievance procedure.
