Provision and Payment of Medical Certificates. a) i) An employee who is absent by reason of illness and whose absence is in excess of three (3) consecutive working days, shall be required to furnish a medical certificate from a duly qualified physician, oral surgeon or chiropractor for each such absence; this certificate is to be submitted to the supervisor/manager by the employee no later than the end of the pay period following that in which the absence occurs.
