In summary, the activities and observations for this quarter consist of the following: • The Groundwater Remedy Optimization (GRO) system operated as planned during this quarter (maintenance was performed on the system that necessitated shutting the system down for brief periods during the reporting period) and pumped approximately 462,000 gallons of water from the AOA • The previous period’s Federal Facility Agreement (FFA) quarterly report was sent to EPA and UDEQ in July 2022 U.S. Department of Energy Monticello NPL Sites FFA Quarterly Report: July 1–September 30, 2022 Doc. The water samples were analyzed for uranium concentrations in each well. Soil and surface water samples were collected from the OU III area in July. The previous period’s Federal Facility Agreement (FFA) quarterly report was sent soil samples ranged from surface to EPA and UDEQ approximately 18 inches in July 2022 depth. Surface water samples were collected from reaches of Montezuma Creek with apparent groundwater intrusion. The data from these samples will be utilized in the characterization of the OU III area. • Xxxxxxx surveillance noted no anomalous conditions for the MVP remedy. The survey measured the horizontal extents and elevations of the following features: ⎯ Top of the well casings (a reference point for depth-to-water measurements) ⎯ Centerline of Montezuma Creek at prescribed intervals ⎯ Water diversions in Montezuma Creek (e.g. culverts, head gates) The area of survey began at U.S. Highway 191 and terminated at the MVP remedy sediment pond to the east. The survey data have been collected using the State Plane Coordinate System instead of the Monticello grid system. • Routine surveillance did not note any violations of MMTS institutional controls (IC’s) that restrict land and groundwater use • Routine surveillance did not note any noted no anomalous conditions for the surface features of the disposal cell and Pond 4, . • Water collection in the engineered solar evaporation pond Pond 4 Leachate Collection and Removal System (LCRS) continued to exceed the action level for July and August. LM has previously notified EPA and UDEQ of this Pond 4 action level exceedance. The water collected in September was below the action level. • Routine surveillance noted no operational operating deficiencies for the Temporary Storage Facility temporary storage facility (TSF) ). LM and Legacy Management Support (LMS) personnel conducted a Facilities Information Management System property asset walkthrough on July 14, 2022 A new powered gate was installed at the access to the Monticello administration parking lot in August. • The Million Gallon Sampling event was conducted on August 2, 2022 • LMS contractor performed the annual site inspection occurred the week of September 510, 2022 2018. The annual site inspection report will be generated in the near future. The site looks good and required site documentation has been maintained. LM hosted a tour at Drought was prevalent in the Monticello National Priorities List (NPL) sites by the International Atomic Energy Association on September 1, 2022area this quarter.

The letters were recommended by EPA and UDEQ in the International Atomic Energy Association July 2022 MMTS CERCLA Five-Year Review report. • Representatives from the Emergency Management group were onsite on September 1December 7, 2022, to conduct an emergency evacuation drill. Due to snowy site conditions, a tabletop discussion was held to simulate the event.
