REPORT OF INJURY. A. At all times, supervisors of all employees must complete the appropriate “Report of Injury” form within twenty-four (24) hours of being notified that a work-related accident has resulted in physical injury to any employee. The supervisor shall provide the employee with a copy of the completed Report of Injury form. B. Any injury suffered by an employee not witnessed by their supervisor, shall be reported in writing by the employee to their supervisor as soon as conditions permit. C. It is the intent of this provision to ensure that staff injuries are reported on a timely basis.
