Resident Referral to Skilled Nursing Unit or Assisted Living Facility. If you currently reside in a continuing care facility or a retirement facility consisting of a nursing home or assisted living facility and residential apartments, this notice applies to you. You may request to be referred to that facility’s skilled nursing unit or assisted living facility. If the request for referral is denied, you may use the appeal process described in Part XIII.
