Revisions to Pro-Forma Call. In Work Availability Form Employees may revise or amend their Pro-Forma Call-In Work Availability Form quarterly. Such revisions shall take effect on the following dates: February 1st, May 1st, August 1st and November 1st. Such revisions shall be submitted no less than twenty-one (21) days prior to the effective revision dates. In addition to the dates specified above, employees may revise or amend their Pro-Forma Call-In Work Availability Form(s) under the following circumstances: - When an employee accepts a permanent part-time position that affects their availability; or - When an employee accepts a temporary position that affects their availability.
