Secondary Notification. Within seven (7) business days of the initial notification, the subrecipient will submit an update to the agency’s Director of Contracts and Quality Assurance and the assigned Contract Manager. The update will include a summary overview of the action taken; any subsequent documentation detailing or clarifying the incident; and a summary of action implemented to date; and other information as may be required. The subrecipient will continue to update the agency on the incident until the incident is resolved. For breach of any data security, or actual or possible unauthorized access to confidential information or client data (collectively called confidential information in this subsection), subrecipient shall notify agency of any unauthorized disclosure, theft, loss, reproduction, or use of any confidential information. The notice shall include, when requested by agency, the name, title, and business address of any person, whether or not personnel of subrecipient, whom subrecipient reasonably believes has unauthorized possession of, or made unauthorized disclosure, reproduction or use of confidential information, along with a detailed description of the confidential information at issue and the factual circumstances surrounding the unauthorized disclosure, theft, or loss.
