Common use of Service Information Clause in Contracts

Service Information. (1) The Provider must have available for Eligible Persons and other interested parties information that describes: (a) the Services the Provider offers; (b) the location of those Services; (c) the hours of access; (d) how to access the Services (eg, whether a referral is required); (e) Service Users’ rights and responsibilities; and (f) any other information necessary to enable Eligible Persons to access the Services.
