Service Order Form. Customers wishing to obtain service from the Company shall submit to the Company a Service Order Form stating the following: i) name or trade name, and address or principal place of business of the Customer; ii) type of service; iii) desired date of commencement of service; and iv) any other information required for provision of International Services.
