Common use of Sick Leave Certificate and Other Medical Forms Clause in Contracts

Sick Leave Certificate and Other Medical Forms. An Employee who is absent for four (4) consecutive days or more will be required to submit a medical certificate from a qualified physician or nurse practitioner at the Employee’s expense. Notwithstanding this obligation, the Employer may request an Employee to provide a medical certificate or any other medical form from a qualified physician or nurse practitioner in other circumstances and for valid reason, in which case the Employer will pay the cost of such certificate to a maximum of the greater of $20.00 or the amount specified in the Ontario Medical Association guidelines for the applicable certificate upon the Employee providing a receipt.
