Subscriber Agreement and Evidence of Coverage. are the PacifiCare documents that describe the costs, benefits or services, procedures, conditions, limitations, exclusions, and other obligations to which Members are entitled and subject to under a Managed Care Plan. A copy of a current standard Subscriber Agreement and Evidence of Coverage for each Managed Care Plan shall be provided to Medical Group by PacifiCare and may be updated from time to time by PacifiCare.
