Tenth Supplemental Indenture. The Issuer issued the Notes under the Base Indenture dated as of August 1, 2011 (the “Base Indenture”) among the HCA Inc., the Guarantors named therein, the Trustee and the Paying Agent, Registrar and Transfer Agent, as supplemented by Supplemental Indenture No. 10, dated as of October 17, 2014 (the “Tenth Supplemental Indenture”), among HCA Inc., the Guarantors named therein, the Trustee and the Paying Agent, Registrar and Transfer Agent. This Note is one of a duly authorized issue of notes of the Issuer designated as its 5.25% Senior Secured Notes due 2025. The Issuer shall be entitled to issue Additional Notes pursuant to Section 2.01 of the Tenth Supplemental Indenture. The terms of the Notes include those stated in the Tenth Supplemental Indenture and those made part of the Tenth Supplemental Indenture by reference to the Trust Indenture Act of 1939, as amended (the “Trust Indenture Act”). The Notes are subject to all such terms, and Holders are referred to the Tenth Supplemental Indenture and such Act for a statement of such terms. To the extent any provision of this Note conflicts with the express provisions of the Tenth Supplemental Indenture or the Base Indenture, the provisions of the Tenth Supplemental Indenture shall govern and be controlling.
