Time of Payment of Claims. Except as provided in this paragraph, benefits payable will be paid immediately after receipt of written proof of loss. Claims for services rendered after expiration of the first month of the grace period for recipients of Advance Payments of the Premium Tax Credit, as set forth in Sections 3.2B. and 4.2B.2., will be pended and will only be paid after the Subscriber makes payment of the Premium due. Any accrued benefits unpaid at the Member’s death shall be paid to the Member’s estate. SAMPLE

If We agree to settle a claim, We shall pay, or shall mail payment, within ten (10) business days, unless a further delay is mandated under an order by a court of competent jurisdiction or required by law. Time of Payment of Claims. All claims and indemnities payable under the terms of the policy shall be paid within thirty (30) days following receipt by Us of due proof of loss. Failure to pay within such period shall entitle You to interest at the rate of 9 percent per annum from the 30th day after receipt of such proof of loss to the date of late payment, provided that interest amounting to less than one dollar need not be paid. You or Your assignee shall be notified by Us of any known failure to provide sufficient documentation for a due proof of loss within thirty (30) days after receipt of the claim. Any required interest payments shall be made within thirty (30) days after the payment. Benefit to Bailee: This insurance will in no way inure to the benefit of any carrier or other bailee.
Time of Payment of Claims. Claims made for indemnities provided under the Policy shall be deemed payable immediately upon receipt of due written proof of loss.

We will review your claim, and if we have all the information, we need we will immediately send you a written decision concerning your claim within thirty (30) days. If we tell you we need more time because of circumstances beyond our control, we may take an additional fifteen (15) days to send you our written decision. If we tell you we need more time and ask you for more information, you will have forty-five (45) days to provide the requested information. We encourage you to send all the requested information at one time, so that we will be able to consider it all when we make our decision. If we do not receive any of the requested information (including documents) within forty-five (45) days, we will make a decision based on the information we have. We will issue our decision within fifteen (15) days of the deadline for receiving the information. If we deny your claim or if we do not pay for all the Services you requested, our written decision will tell you why we denied your claim and how you can appeal.
Time of Payment of Claims. Benefits payable under the Policy for any loss incurred will be paid within 30 days of our receipt of written Proof of Loss.
