Total Health Incentive Plan. In 2015, the parties amended the structure of the Total Health Incentive Plan, including biometric screening and outcome measures. The THIP payout will be $500 per employee per year accordingly: » Payout One “Gate” – In each region, employees are eligible for an annual payout of $150 upon the region achieving the biometric screening percentages. » Payout Two “Outcomes”– If a region meets the screening percentages established for the Gate, employees in that region are eligible for a second annual payout of $350 for health outcome improvements achieved by the region. To set the biometric screening and outcome measures in more detail, the parties will conduct an analysis of the current state, current and planned interventions, and the number of individuals we need to impact in each region. Members of the joint team will include representatives of management (including a representative from CMI, finance, clinical operations, communications and CAR) and representatives of labor, which will choose its own members. The collection and analysis of the data will begin immediately and will be utilized in setting screening and outcome measures as soon as possible, but no later than the end of September 2015. The joint team will recommend the appropriate biometric screening percentages and outcome improvement measures by the end of October 2015. Exhibit I.H.2.d. contains the elements of the Gate and the regional outcome improvement measures that are each eligible for payout.
