Transition of Care Requirements for New Members. The MCP shall follow the transition of care requirements as outlined below for any new member, regardless of if the individual is transitioning from FFS or another MCP. a. Upon notification from ODM that an individual will be switching to a different MCP or MyCare Ohio plan, the disenrolling MCP shall provide specific information related to the disenrolling member to the enrolling plan as specified by ODM.
