Common use of Transport of Patients Clause in Contracts

Transport of Patients. 19.1 The Provider agrees to transport or bear the cost of transport charges to the hospital and back after the surgery is conducted on the Beneficiary and obtain acknowledgement from the patient accordingly. The acknowledgement sheet generated from the portal need to be signed by the Patient and the signed copy should be scanned and uploaded to the Trust portal.
