Transportation Requirement. A transportation log must be included in the consumer chart for MSHN women’s specialty designated Providers, MSHN outpatient Providers and other qualified providers whose consumers are utilizing transportation services. In accordance with the Medicaid Provider Manual - Non-Emergency Medical Transportation, individuals transporting consumers must hold a valid driver's license appropriate to the class of vehicle being operated as defined by the Michigan Vehicle Code Act 300 of 1949. Transportation needs must be identified in the Plan of Service and clearly documented within the consumer’s individualized treatment plan.
