Trauma Informed Care Organizational Survey. PROVIDER under contract with MSHN shall complete the Trauma Informed Care (TIC) Organizational Survey. This document shall be uploaded to Box, with supporting documentation if necessary. PROVIDER shall set goals annually and report progress. The Trauma Informed Organizational Survey will be utilized as a self-assessment and completed every three years.
