Common use of Unassigned Patient Clause in Contracts

Unassigned Patient. A patient who does not identify a physician in the community and on the Medical Staff who is taking care of their medical needs. If a patient has a continuity relationship with a primary care physician (Internal Medicine, Family Practice, Pediatrics, or Obstetrician/Gynecologist) then they are to be considered “assigned”. (Note: For “assigned” patients, the Primary Care Physician should be notified if a patient is admitted to Hospital.)
