Common use of Voluntary Discharge Clause in Contracts

Voluntary Discharge. If the Resident no longer requires the services provided by the Facility, or voluntarily wishes to be discharged, the Resident, Resident Representative and Sponsor will cooperate with the Facility in the development and implementation of a safe, appropriate, and timely discharge plan.

Voluntary Discharge. If the Resident no longer requires the services provided by the Facility, or voluntarily wishes to be discharged, the Resident, Resident Representative and Sponsor will agrees to cooperate fully with the Facility in the development and implementation of a safe, appropriate, and timely discharge plan.

If the Resident no longer requires the services provided by the Facility, or voluntarily wishes to be discharged, the Resident, Resident Representative Designated Representative, Sponsor and Sponsor Facility will cooperate with the Facility in the development and implementation of a safe, appropriate, and timely discharge plan.

If the Resident no longer requires the services provided by the Facility, or voluntarily wishes to be discharged, the Resident, Resident and Resident Representative and Sponsor will agree to cooperate fully with the Facility in the development and implementation of a safe, appropriate, and timely discharge plan. The Resident will be informed of his or her due process rights in the event that the Facility initiates a transfer or discharge and may appeal the Facility’s determination in accordance with applicable regulations.
