Xxxxxx of Termination. ‌ The Contractor shall provide written notice of termination of a Participating PCP, within 15 days after receipt or issuance of the termination notice, to each Enrollee who received his or her Primary Care from the terminated PCP. Such written notice shall describe how the Enrollee’s continuing need for services shall be met. Whenever possible, such notice shall be provided to Enrollees 30 days prior to such Provider termination;
