Common use of XXXXXXX’S RESPONSIBILITIES Clause in Contracts

XXXXXXX’S RESPONSIBILITIES. Under this Grant Agreement, Grantee will perform the responsibilities listed below. A. Grantee will provide services to the following eligible population(s): Children, their Caregivers, and individuals working with Children or Caregivers of Children that demonstrate atypical social or emotional development or other challenging behavior. B. Grantee will provide services in the following Community(ies): Travis County. C. Grantee will use funds under this Grant Agreement for activities and costs associated with implementing Grantee’s innovative strategies to provide the following to the eligible Participants: 1. Resiliency, coping, and social skills; 2. An increase in healthy social and familial relationships; and 3. Support for building parenting skills and behaviors. D. Grantee will engage in the following activities to achieve the results identified in Section III(C) of this Attachment A, Statement of Work: education, skill-building, training, outreach and awareness, and provision of goods or services to individuals, families or groups to meet required outcomes. E. Xxxxxxx’s activities identified at Section III(D) of this Attachment A, Statement of Work, should demonstrate progress directly, or by proxy towards outcomes including, but not limited to the following: 1. Improving Children’s relationship skills; 2. Improving Children’s self-esteem; 3. Reducing involvement in the juvenile justice system; 4. Reducing xxxxxx care placement and/or relinquishment (e.g., expand participation in the relinquishment avoidance program under Texas Family Code, Chapter 262, Subchapter E); 5. Reducing emergency room use for behavioral health needs; 6. Reducing admissions to State hospitals, inpatient mental health facilities, and residential behavioral health facilities; and 7. Reducing the need for future intensive mental health services. F. Grantee will ensure that all of its Grant Projects meet the following SMART Innovation Grant Program requirements: 1. Use of a local approach; 2. Provide personalized support to Participants; and 3. Target the populations identified in Section III(A) of this Attachment A, Statement of Work. G. Grantee will comply with the Match requirements for the SMART Innovation Grant Program identified at Attachment B, Budget and Invoice Requirements, and Section 5.4, Cost Sharing or Matching Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda. H. Grantee will comply with the Participant requirements for the SMART Innovation Grant Program identified at Section 2.6.1, Participant Related Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. I. If Grantee administers a Participant intake process or a tool that assesses the needs of Participants, it will comply with the intake requirements for the SMART Innovation Grant Program identified at Section 2.6.2, Intake Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. J. Grantee will develop and implement Participant evaluations that assess the effectiveness of the services provided and will comply with the Participant evaluation requirements for the SMART Innovation Grant Program identified at Section 2.6.3, Participant Evaluation Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. K. Grantee will comply with the requirements for developing or providing information or educational written materials for the SMART Innovation Grant Program identified at Section 2.6.4, Informational or Education Material Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. L. Grantee will develop and maintain contingency plans to ensure personnel continuity in accordance with the contingency plan requirements for the SMART Innovation Grant Program identified at Section 2.6.5, Contingency Plan Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. M. Grantee will ensure that its Project staff meet the staffing requirements for the SMART Innovation Grant Program identified at Section 2.6.6, Staffing Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. N. Grantee will be responsible for monitoring the effectiveness of its Projects and the quality of all its services under this Grant Agreement and will comply with the self-monitoring requirements for the SMART Innovation Grant Program identified at Section 2.6.7, Self-Monitoring Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda, and require its subgrantees to adhere to the same. O. Xxxxxxx will submit a performance report, on a quarterly basis. Specifically, the Quarterly Performance Report will be submitted, via email, no later than the last Business Day of the month following the end of each State fiscal quarter to the following email addresses: XXXXXxxxxXxxxxxxxx@xxx.xxxxx.xxx; and the HHSC Contract Representative identified at Section VII, Contract Representatives, of this Grant Agreement. The Quarterly Performance Report will require Grantee to report on the following measures: 1. Number of unique Participants receiving a minimum of one (1) grant-funded service. Grantee is required to demonstrate by the end of the State Fiscal Year that at least ninety percent (90%) of its projected number of Participants were served; 2. Percentage of unique Participants receiving a minimum of one (1) grant-funded service within the State Fiscal Year that report satisfaction with the service(s). Grantee is required to demonstrate by the end of the State Fiscal Year that at least seventy-five (75%) of Participants completing a post-assessment (see Section 2.6.3, Participant Evaluation Requirements, of Attachment H, HHSC RFA No. HHS0013881, Including All Addenda) report satisfaction with the service(s); 3. Xxxxxxx’s identified performance measures and targets as identified at Section IV(B) of this Attachment A, Statement of Work; and 4. Narrative items detailing the following information: a. Completed Grant Project activities, e.g., staffing, program activity, Community collaboration; b. Challenges encountered when implementing and operating the SMART Innovation Grant Program, and plans for overcoming those challenges; and c. Real or perceived benefits related to the services provided to Participants and/or the Community. Perceived benefits may include Participant success stories, staff input, Community response, or other material. Grantee must include Outcome data for services and activities, if available. P. Grantee will submit a second performance report, on a biannual basis.
