Common use of Your HSA Website Account Clause in Contracts

Your HSA Website Account. You will require access to the internet to open your HSA. Your HSA Administrator will provide you with access to a personal HSA website account. You will need to establish a user name and password. Your HSA Administrator will post all information you need to manage your HSA on your HSA website account. This information includes your account balance, contributions, distributions, annual IRS Forms 1099-SA and 5498-SA, and any amendments to this agreement. You agree to review your HSA at least once per month. You are responsible for protecting access to your HSA and not sharing with anyone your username and password. Your HSA website account has all of the information and tools you need to manage your HSA and make investments.
