Related to Affiliated Optometrist

Optometrist means each individually ----------- licensed doctor of optometry who is employed or otherwise retained by or associated with Practice, each of whom shall meet at all times the qualifications described in Sections 5.2 and 5.3 hereof.
Participating Optometrist means an Optometrist who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.
Non-Participating Optometrist means an Optometrist who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.
Ophthalmologist means each individually licensed Ophthalmologist, if any, who is employed or otherwise retained by or associated with the Practice, each of whom shall meet at all times the qualifications described in Section 4.3 and Section 4.4.
