Benefit Guide definition

Benefit Guide means the description of benefits accessible at www.cnbankerrewards.com/benefitguide or obtained via written request sent to our Contact Address. The Benefit Guide is incorporated by reference into the MRPC Program Terms.
Benefit Guide means; (a) the Referral Benefit information page at https://help.acuityscheduling.com/hc/articles/34121376720781 and (b) solely if applicable to you, any Promotion Guides (as defined below) related to a Promotion (as defined below) for which you are an Eligible Promotion Participant (as defined below).

Examples of Benefit Guide in a sentence

  • The specific terms of coverage, exclusions and limitations are contained in the Summary Plan Description, the Group Benefit Guide or the Group Service Agreement.

  • A description of this benefit, which may change from time to time, is set forth in the Resident Benefit Guide.

  • Special enrollment periods shall be provided in accordance with the terms and conditions of the Blue Cross/Blue Shield Benefit Guide.

  • To the extent not specifically set forth in this Basic SPD and Plan document and Adoption Agreement, the Benefit Guide may contain additional eligibility conditions to participate, and will contain a specific explanation of covered and excluded benefits (e.g. schedules of benefits), cost-sharing requirements, network requirements, other terms and conditions for receipt of benefits and the name and contact information for the insurance carrier and/or any other third party administrator.

  • Information about the EAP program and how to access it will be provided in the annual Benefit Guide.

  • Guaranteed issue amount and further details regarding the option to purchased supplemental life insurance, may be found in the SSA Benefit Guide which is distributed to employees upon hire and during annual open enrollment.
