Care Transitions means a post-discharge program aimed at reducing inpatient readmission by monitoring the Covered Person’s compliance with discharge orders. The program includes a Home Visit within two days of discharge, a Pharmacy review, a minimum of three telephonic follow-up calls with a health coach, and coordination of services with Physicians/Providers.
