Transition Plan definition

Transition Plan means a transition plan, acceptable to the LHIN that indicates how the needs of the HSP’s clients will be met following the termination of this Agreement and how the transition of the clients to new service providers will be effected in a timely manner; and
Transition Plan means a written plan for the period of time between an individual's entry into a particular service and when the individual's ISP is developed and approved by the ISP team. The plan must include a summary of the services necessary to facilitate adjustment to the services offered, the supports necessary to ensure health and safety, and the assessments and consultations necessary for the ISP development.
Transition Plan has the meaning set forth in Section 3.1.

Examples of Transition Plan in a sentence

  • If the HSP fails to provide the required 6 months’ Notice that it intends to allow this Agreement to expire, or fails to provide a Transition Plan along with any such Notice, this Agreement shall automatically be extended and the HSP will continue to provide the Services under this Agreement for so long as the Funder may reasonably require to enable all clients of the HSP to transition to new service providers.

  • In the event of termination by the Funder pursuant to this section, the Funder and the HSP will develop a Transition Plan.

  • If the HSP intends to allow this Agreement to expire at the end of its term, the HSP will provide 6 months’ Notice (or such shorter period as may be agreed by the HSP and the Funder) to the Funder, along with a Transition Plan, acceptable to the Funder, that indicates how the needs of the HSP’s clients will be met following the expiry and how the transition of the clients to new service providers will be effected within the 6- month Notice period.

  • The Maintenance Contractor shall develop a Maintenance Transition Plan as set forth in Section 3.6.1.

  • The LEA shall provide CONTRACTOR with a copy of the IEP including the Individualized Transition Plan (hereinafter referred to as “ITP”) of each student served by CONTRACTOR.

Transition Plan means a plan that is personalized at the direction of the youth that:
Transition Plan means a written plan for the period of time between an individual's entry into a particular service and when the individual's Individual Support Plan is developed and approved by the Individual Support Plan team. The transition plan includes a summary of the services necessary to facilitate adjustment to the services offered, the supports necessary to ensure health and safety, and the assessments and consultations necessary for the Individual Support Plan development.
Transition Plan means the Contractor's plan for the Lead-in Period forming part of the Contract.
Transition Plan has the meaning set forth in Section 6.4(c).
Transition Plan shall have the meaning provided in Section 4.3.
Transition Plan means, when used in relation to a youth in the custody of the secretary, an individualized strategy for the provision of medical, mental health, education, employment and housing supports as needed for the adult and, if applicable, for any minor child of the adult, to live independently and specifically provides for the supports and any services for which an adult with a disability is eligible including, but not limited to, funding for home and community based services waivers.
Transition Plan means a written case plan that is personalized, as detailed as the child may elect, and that includes specific options on housing, health insurance, education, local opportunities for mentors and for continuing support services, work force supports, and employment services. A transition plan also must include information about the importance of designating another individual to make health care treatment decisions on behalf of the child if the child becomes unable to participate in such decisions and the child does not have, or does not want, a relative who would otherwise be authorized to make such decisions, and provides the child with the option to execute a health care power of attorney or health care proxy.
