Converted facility definition

Converted facility means an intermediate care facility for individuals with intellectual disabilities or a former intermediate care facility for individuals with intellectual disabilities that converted some or all of its beds to providing home and community-based services funded by the individual options waiver pursuant to section 5124.60 of the Revised Code.
Converted facility means the entity that exists after the facility is converted under this part.
