Designated Transplant Facility definition

Designated Transplant Facility a Hospital or Alternate Facility that has entered into an agreement with or on Our behalf to render Health Services for Covered transplants. The facility may or may not be located within Our Service Area. "Durable Medical Equipment" or "DME" - medical equipment that meets the following requirements.
Designated Transplant Facility means a facility that has agreed to provide Approved Transplant Services to Covered Persons pursuant to an agreement with a transplant provider network or rental network with which the Plan has a contract.
Designated Transplant Facility means a facility which has agreed to provide Approved Transplant Services to participants pursuant to either an agreement with the Plan, or an agreement with a transplant provider network with which the Plan has a contract.

Examples of Designated Transplant Facility in a sentence

  • Covered Services for patient selection criteria shall be covered at one (1) Designated Transplant Facility.

  • If the second Designated Transplant Facility also determines the Member is not eligible for the transplant procedure, no coverage will be provided for further transplant-related services.
