Former Provider definition

Former Provider means the provider previously appointed by the Council toprovide all or any of the services which are substantially similar to any of the services prior to the appointment of the Provider;
Former Provider means the person reimbursed by the department for the provision of health care services at a nursing home, boarding home, hospital or other provider of health care services prior to its transfer.[PL 2011, c. 687, §5 (AMD).]
Former Provider means a provider supplying the services to the Service Purchaser before the Relevant Transfer Date that are the same as or substantially similar to the Services (or any part of the Services) and shall include any sub- contractor of such Provider (or any sub-contractor of any such sub- contractor)

  • If for any reason after the Relevant Transfer Date the Provider ceases to be an Admission Body other than on the date of termination or expiry of this Contract or because it ceases to employ any Transferring Former Provider LGPS Employees, then the Provider shall establish a registered pension scheme which is certified by the Government Actuary’s Department as providing benefits which are the same as, broadly comparable to or better than those benefits provided by the LGPS at the Relevant Transfer Date.

  • C10.1. Subject to clause C10.2, the Authority shall procure that each Former Provider shall indemnify the Provider and any Notified Sub-Contractor against any Employee Liabilities in respect of any Relevant Employee (or, where applicable any employee representative as defined in the TUPE) under TUPE, to the extent that such loss relates to the period of time that the Relevant Employee was employed by the Former Provider.

  • Provider of health care means a physician or physician assistant licensed pursuant to chapter 630, 630A or 633 of NRS, perfusionist, dentist, licensed nurse, dispensing optician, optometrist, practitioner of respiratory care, registered physical therapist, podiatric physician, licensed psychologist, licensed audiologist, licensed speech-language pathologist, licensed hearing aid specialist, licensed marriage and family therapist, licensed clinical professional counselor, chiropractor, licensed dietitian or doctor of Oriental medicine in any form.

  • Non-Participating Home Infusion Therapy Provider means a Home Infusion Therapy Provider who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Health plan issuer means an entity subject to the insurance laws and rules of this state, or subject to the jurisdiction of the superintendent of insurance, that contracts, or offers to contract to provide, deliver, arrange for, pay for, or reimburse any of the costs of health care services under a health benefit plan, including a sickness and accident insurance company, a health insuring corporation, a fraternal benefit society, a self-funded multiple employer welfare arrangement, or a nonfederal, government health plan. “Health plan issuer” includes a third party administrator to the extent that the benefits that such an entity is contracted to administer under a health benefit plan are subject to the insurance laws and rules of this state or subject to the jurisdiction of the superintendent. The “Health plan issuer” is also called the Administrator in this Benefit Booklet.

  • Employed by, or provide service to, the Company means employment or service as an employee or director of the Company.

  • Provider Group means a medical group, independent

  • Provider Service means a Provider’s hosted online services (if any) as described in the Solution Exhibit which is provided by Provider to Customers located in the Territory through remote access via the Internet as part of the BPO Service. A Provider Platform Application shall be considered a Provider Service.
