Formulary List definition
Formulary List means the listing of preferred and non-preferred medications as determined by AvMed’s Pharmacy and Therapeutics Committee based on the clinical efficacy, relative safety and cost in comparison to similar medications within a therapeutic class. This multi-tiered list establishes different levels of cost-sharing for medications within therapeutic classes. As new medications become available, they may be considered excluded until they have been reviewed by ▇▇▇▇▇’s Pharmacy and Therapeutics Committee. Specific medications on the Formulary List and their placement in a given therapeutic class are subject to change at any time without prior notice to you or your approval. It is your responsibility to consult with your Attending Physician to determine whether a medication is on the Formulary List at the time the prescription is rendered. Examples of Formulary List in a sentence
The copayment structure is as follows: $15 Generic $35 Brand Name Drug on the Formulary List –“Preferred Brands” $55 Brand Name Drug NOT on the Formulary List – “Non-Preferred Brands” $100 Drugs Classified as Specialty Members are required to participate in a mandatory mail order program that will include a 90 day supply with co-pays at two (2) times the above amounts.
MEDICAL/DENTAL BENEFITS: The Company will provide each employee with a Drug Formulary List and Benefit Booklet.
