High Medical Costs means: To include annual out of pocket costs at the hospital that exceed the lesser of 10% of the patient’s current family income or the family income in the prior 12 months.

High Medical Costs means the annual out-of-pocket costs of a patient whose family income exceeds the Organization’s thresholds for financial assistance. These costs are considered if they are either: (i) incurred by the patient at the Organization’s hospital that exceed the lesser of the patient’s current family income or family income in the prior twelve (12) months, or (ii) out-of- pocket medical expenses that exceed ten percent (10%) of the patient’s family income, if thepatient provides documentation of the patient’s medical expenses paid by the patient or the patient’s family in the prior twelve (12) months.

Examples of High Medical Costs in a sentence

  • Each bid shall specify whether it is for an on-peak weekend on-peak, weekday on-peak, off-peak, or 24-hour period.

  • Medical care means amounts paid for:

  • Emergency medical technician means a person who is either an EMT-I, EMT-II, or EMT-P (paramedic), and possesses a valid certificate or license in accordance with the standards of Division 2.5 (commencing with Section 1797) of the Health and Safety Code.

  • Medical home means a team approach to providing health care that originates in a primary care setting; fosters a partnership among the patient, the personal provider, other health care professionals, and where appropriate, the patient’s family; utilizes the partnership to access all medical and nonmedical health-related services needed by the patient and the patient’s family to achieve maximum health potential; maintains a centralized, comprehensive record of all health-related services to promote continuity of care; and has all of the characteristics specified in Iowa Code section 135.158.
