Generic form means an electronic or paper form that is not prescribed by a particular municipal corporation and that is designed for reporting taxes withheld by an employer, agent of an employer, or other payer, estimated municipal income taxes, or annual municipal income tax liability or for filing a refund claim.
Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.
Electronic form means an automated system transmitting information electronically according to the Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.
Letter of Transmittal and Election Form means the letter of transmittal and election form for use by the Corporation Shareholders with respect to the Arrangement;
Proper Form means a payment sent by mail or courier that:
Book entry form or “book entry system” means a form or system under which (i) the ownership of book entry interests in Bonds and the principal of and interest on the Bonds may be transferred only through a book entry and (ii) physical Bond certificates in fully registered form are issued by the City only to a Depository or its nominee as registered owner, with the Bonds deposited and maintained in the custody of the Depository or its agent. The book entry maintained by others than the City is the record that identifies the owners of book entry interests in those Bonds and that principal and interest.
Form means a form appended to these rules;
approved form means a form approved by the Minister;
Application Form means any application form to be completed by subscribers for Shares as prescribed by the Company from time to time.
Exhibit 2 Standard File Codes – Delinquency Reporting
Transfer Form means a standardized form prescribed by the Management Company to be duly filed by the investor to transfer Units and will be stated in this Offering Document.
Database application form means the application form required by the Hibiscus Coast Municipality to be filled in by the successful tenderer, following the award of the contract, for inclusion on the HCM database before payment is made.
Certificate form means the form on which the certificate is delivered or issued for delivery by the issuer.
Assignment Details Form means written confirmation of the assignment details to be given to the Agency Worker upon acceptance of the Assignment;
Policy form means the form on which the policy is delivered or issued for delivery by the issuer.
Exhibit B means [Vendor]’s Response.
Proposal Form means that certain Part C of the RFP, completed and executed by the Proponent, to which this Appendix 1 is appended.
Transmittal Letter has the meaning specified in Section 3.1(a).
Special Instruction Form means a standardized form prescribed by the Management Company to be duly filled by the investor to change his/her particulars and will be stated in this Offering Document.
Authorization Form means a form specified in s. 440.73.
Variation Form means the form that will be completed and signed by the Parties to effect a Variation which shall be in the form set out in Framework Schedule 19 (Variation Form);
Exhibit 1 means Exhibit 1 to this Schedule C.
Uniform business entity application means the current version of the NAIC Uniform Business Entity Application for resident and nonresident business entities.
Bid Application Form means the form as specified in Annexure II of this E-Auction Process Information Document;
Forms or "form" means both paper forms and forms completed electronically.
Assignment Form means the assignment form attached as Annex 2 hereto.
