Mail Order Pharmacy Program definition

Mail Order Pharmacy Program means the program described in Section 20 through which Eligible Persons may submit a prescription along with the applicable Copayment/Coinsurance to Express Scripts for dispensing via mail order.
Mail Order Pharmacy Program means the program under which Members may purchase Formulary products from an Xxxxx contracted mail order pharmacy for home delivery upon verification of eligibility and payment of the applicable Copayment, and the claim is submitted by the Participating Pharmacy to Xxxxx for payment in accordance with this Agreement and the applicable Participating Pharmacy agreement.

Examples of Mail Order Pharmacy Program in a sentence

  • Pharmacy Benefit Manager will provide a Mail Order Pharmacy Program as further described in Exhibit A.

  • Xxxxx shall require Participating Pharmacies to comply with Xxxxx’x terms and conditions applicable to participation in the Retail Pharmacy Program or Mail Order Pharmacy Program, as well as all applicable state board of pharmacy rules and regulations.

  • Express Scripts will credit CITY OF TEMPE, within ninety (90) days following the end of each calendar quarter the sum of (i) $32.00 times the total number of Covered Brand Drug prescriptions billed and paid for under CITY OF TEMPE’s Retail Pharmacy Program during such Contract Year, plus (ii) $135.00 times the total number of Covered Brand Drug prescriptions billed and paid for under CITY OF TEMPE’s Mail Order Pharmacy Program during the same Contract Year (collectively the “Guaranteed Rebates”).

  • The Program Pricing Terms applicable to the Mail Order Pharmacy Program are set forth in Schedule A (which is considered confidential information), in addition to the performance standards and penalties set forth in this agreement.

  • Notwithstanding anything herein to the contrary, prescriptions filled under the Mail Order Pharmacy Program for less than a forty-five (45) day supply shall be considered to be Retail Pharmacy program claims for purposes of calculating administrative fees under this Section 4.

  • The Copayment/Coinsurance amount for each prescription or refill dispensed by a Medco mail order pharmacy under the Mail Order Pharmacy Program shall be as designated for each Group in the applicable Plan Design(s).

  • The Program coverage (Covered Drugs/Exclusions) and days supply limitation under the Mail Order Pharmacy Program will be as designated by CITY OF TEMPE in the applicable Plan Design.

  • Mail order pharmacy means a pharmacy licensed by this

  • Diagnostic clinical procedures manual means a collection of written procedures that describes each method (and other instructions and precautions) by which the licensee performs diagnostic clinical procedures; where each diagnostic clinical procedure has been approved by the authorized user and includes the radiopharmaceutical, dosage, and route of administration.

  • Chain pharmacy warehouse means a physical location for prescription drugs that acts as a central warehouse and performs intracompany sales or transfers of the prescription drugs to a group of chain pharmacies that have the same common ownership and control.

  • Drug Master File or “DMF” is described in 21 C.F.R. Part 314.420. A DMF is a submission to the FDA that may be used to provide confidential detailed information about facilities, processes, or articles used in the manufacturing, processing, packaging, and storing of one or more human drugs.

  • Retail pharmacy means a pharmaceutical facility dispensing prescription drugs and devices to the general public.

  • Network pharmacy means any pharmacy that has an agreement to accept our pharmacy allowance for prescription drugs and diabetic equipment/supplies covered under this agreement.
