Physician Claim definition

Physician Claim means any Other Claim asserted by a Physician.
Physician Claim means a claim submitted by a Physician for reimbursement of Medical 32 Services.
Physician Claim means a claim submitted by a physician or clinic for reimbursement of 11 Medical Services and shall not include Clinic Claims for Enhanced Primary Care Services as defined in 12 the MSI Clinic Agreement.

Examples of Physician Claim in a sentence

  • The holder of a Physician Claim who elects the additional discount but fails to tender all undisputed sums to the Debtors as required under Section 8.8.5 of this Plan shall not be entitled to the additional discount.

  • No distributions shall be made on account of a Disputed Physician Claim until such Claim becomes Allowed.

  • A Notice of Buyout Election submitted by the holder of a Physician Claim who filed a proof of claim who votes to reject the Plan will be deemed invalid and of no force or effect.

  • Professional Claim means an Administrative Claim of a Professional for compensation for services rendered or reimbursement of costs, expenses, or other charges and disbursements incurred relating to services rendered or expenses incurred after the Petition Date and prior to and including the Confirmation Date.

  • Specialist Physician means a licensed physician who qualifies as an attending physician and who examines a patient at the request of the attending physician or authorized nurse practitioner to aid in evaluation of disability, diagnosis, or provide temporary specialized treatment. A specialist physician may provide specialized treatment for the compensable injury or illness and give advice or an opinion regarding the treatment being rendered, or considered, for a patient’s compensable injury.

  • Clean Claim means a claim that:

  • Treating physician means an individual who:

  • Bona fide physician-patient relationship means a treatment or counseling relationship between a physician and patient in which all of the following are present:

  • Urgent Care Claim means a claim for medical care or treatment where making a non-urgent care decision: (a) could seriously jeopardize the life or health of the claimant or the ability of the claimant to regain maximum function, as determined by an individual acting on behalf of the plan applying the judgment of a prudent layperson who possesses an average knowledge of health and medicine; or (b) in the opinion of a physician with knowledge of the claimant’s medical condition, would subject the claimant to severe pain that cannot be adequately managed without the care.

  • Health professional means any of the following:

  • Licensed mental health professional or "LMHP" means a physician, licensed clinical psychologist, licensed professional counselor, licensed clinical social worker, licensed substance abuse treatment practitioner, licensed marriage and family therapist, certified psychiatric clinical nurse specialist, licensed behavior analyst, or licensed psychiatric/mental health nurse practitioner.

  • Qualified mental health professional means a licensed medical practitioner or any other person meeting the qualifications specified in OAR 309-019-0125.

  • Psychiatrist means an individual who meets the minimum professional and licensure requirements set forth in Title 9, CCR, Section 623.

  • Professional Fee Claim means a Claim by a professional seeking an award by the Bankruptcy Court of compensation for services rendered or reimbursement of expenses incurred through and including the Confirmation Date under sections 330, 331, 503(b)(2), 503(b)(3), 503(b)(4), or 503(b)(5) of the Bankruptcy Code.

  • Child psychiatrist means a person having a license as a
