Reimbursement Request definition

Reimbursement Request shall have the meaning set forth in Section 3.10(a).

Reimbursement Request means the Recipient’s request for Project Funds from the State Water Board as set forth in Exhibit B.

Reimbursement Request means a properly completed written request to the City in a form satisfactory to the City requesting a disbursement of the Funding to reimburse Funding Recipient for Eligible Costs incurred and paid by Funding Recipient during the period covered by said Reimbursement Request. Each Reimbursement Request must be executed by an authorized representative of Funding Recipient.

• This Account-Based Offerings Administrative Services Agreement will be considered accepted and binding by the parties when you sign your Group Setup Form or, when applicable, your Claim Reimbursement Request Form, or you pay your first month’s service fees for Account-Based Offerings.

• When you elect to have Blue Cross and Blue Shield work with your designated vendor for your Account-Based Offerings, Blue Cross and Blue Shield will provide services as described in this Account-Based Offerings Administrative Services Agreement and in your Claim Reimbursement Request Form.

More Definitions of Reimbursement Request

Reimbursement Request means a reimbursement request signed by the Borrower, substantially in the form set out in Part 1 of Schedule 3 (Requests and Notices).
