State Utilization Data definition

State Utilization Data means the data used by the Department to reimburse pharmacy providers under the Wyoming Medicaid Program. State Utilization Data excludes data from covered entities identified in Title 42 USC 256b(a)(4) in accordance with Title 42 USC 256b(a)(4)(A) and 1396r-8(a)(5)(C).

State Utilization Data means the data used by Participating States to reimburse pharmacy providers under Participating States' Medicaid Program (and other non-Medicaid programs approved by CMS in the state plan(s) as provided in Section 2.1 hereof). • In the event that in any quarter a discrepancy in the State's State Utilization Data is questioned by the Manufacturer, which the Manufacturer and the State in good faith are unable to resolve, the Manufacturer will provide written notice of the discrepancy to the State.

• KDHE-DHCF shall deliver the State Utilization Data to CONTRACTOR in the same manner, and at the same time that State Utilization Data is submitted under Kansas Medicaid.

More Definitions of State Utilization Data

State Utilization Data means the data used by KDHE-DHCF to reimburse pharmacy providers under the Kansas Medicaid Program. State Utilization Data excludes data from covered entities identified in Title 42 U.S.C. Sec. 256b(a)(4) in accordance with Title 42 U.S.C. Sec. 256b(a)(4)(A) and 1396r-8(a)(5)(C). State Utilization shall not include any products dispensed to Medicaid recipients by pharmacies outside of the United States.

State Utilization Data means the OHCA data which reflects reimbursement to pharmacy and medical providers for Contracted Products under the Oklahoma Medicaid Program. Medicaid Utilization Information to be supplied includes, for each NDC number or Unique Product Identifier (UDI): 1) Product name; 2) Units; 3) Number of prescriptions; and 4) Total amount reimbursed. A zero value in the total amount reimbursed field could be appropriate for MCO data only. State Utilization Data excludes data from covered entities identified in Title 42 USC 256b(g)(4) in accordance with Title 42 USC 256b(a)(4)(A) and 1396r-8(a)(5)(C). State Utilization Data shall not include any Products dispensed to Medicaid Members by Pharmacies outside of the United States. Fee-for-Service utilization information is based on claims paid during a Quarter. Managed Care Plan Utilization Data is based on claims dispensed during the Quarter for Contracted Products.

State Utilization Data means the OHCA data which reflects reimbursement to pharmacy providers under the Oklahoma Medicaid Program. State Utilization Data excludes data from covered entities identified in Title 42 USC 256b(g)(4) in accordance with Title 42 USC 256b(a)(4)(A) and 1396r- 8(a)(5)(C). State Utilization Data shall not include any Products dispensed to Medicaid Recipients by Pharmacies outside of the United States.

State Utilization Data means the data derived from reimbursement by the State and its Contractors to pharmacy providers under the Arizona Medicaid Program. State Utilization Data excludes data from covered entities identified in Title 42 U.S.C. §256b(a)(4) in accordance with Title 42 U.S.C. §256b(a)(5)(A) and 1396r-8(a)(5)(C).

State Utilization Data means the data used by the State to reimburse Participating Providers under the State’s Medicaid Program (and other state funded approved programs). State Utilization Data excludes data from covered entities identified in Title 42 U.S.C. §256b (a) (4) in accordance with Title 42 V.S.C. §256b (a) (5) (A) and 1396r-8(a) (5) (C).

State Utilization Data means the data used by the State to reimburse State of Vermont Medicaid providers under the State’s Medicaid Program. State Utilization Data excludes data from covered entities identified in Title 42 U.S.C.§256b(a)(4) in accordance with Title 42 V.S.C. §256b(a)(5)(A) and 1396r-8(a)(5)(C).
