VIVA HEALTH definition

VIVA HEALTH means VIVA HEALTH, Inc. an Alabama corporation licensed as a health maintenance organization. VIVA HEALTH may subcontract with other companies as it deems necessary to carry out the terms of this Certificate.

  • A family member, friend, provider, or any other person may act on behalf of the Member after written notification of authorization is received by VIVA HEALTH from the Member.

  • All surgical services must have authorization from VIVA HEALTH prior to the surgical procedure.

  • VIVA HEALTH asks Members to try this process first to resolve any problems.

  • Services received between the date a Member’s coverage is terminated by the Employer and the date VIVA HEALTH is notified by the Employer of the termination are not Covered Services even when such services have been authorized by VIVA HEALTH or a Participating Provider.

  • Likewise, if benefit coverage is limited to a specified dollar amount, services received for the benefit after the specified dollar amount is reached are not Covered Services even if the services are authorized by VIVA HEALTH.

  • The medical care provided to you by your Personal Care Provider, specialists, or other health care professionals will be reviewed by VIVA HEALTH for eligibility, coverage, and Medical Necessity.

  • In addition, unresolved concerns about informed consent for or refusal of health services may, at times, prove to be a barrier for some people with intellectual disabilities (OZDonnell, 1994).

  • Except for Emergency Services as set forth in Part IX.D. or as otherwise prior-approved by VIVA HEALTH, all services must be provided by a Participating Provider in this Plan’s Provider System, subject to the limitations set forth in Part IX and X and the limitations, Cost Sharing, and any Lifetime maximum set forth in Attachment A.

  • VIVA HEALTH has available an independent external review process for denied claims for benefits.

  • UConn Health or “UCH” shall mean University of Connecticut Health and its affiliates.

  • Home health aide means an individual employed by a home health agency to provide home health services under the direction of a registered nurse or therapist.

  • Home health aide services means the personal care and maintenance activities provided to individuals for the purpose of promoting normal standards of health and hygiene.

  • Home health services means part-time or intermittent skilled nursing services, other therapeutic services (physical therapy, occupational therapy, speech therapy), and home health aide services made available on a visiting basis in a place of residence used as the client's home.

  • Behavioral health means the promotion of mental health, resilience and wellbeing; the treatment of mental and substance use disorders; and the support of those who experience and/or are in recovery from these conditions, along with their families and communities.

  • Home health agency means a person certified by medicare whose business is to provide to individuals in their places of residence other than in a hospital, nursing home, or county medical care facility 1 or more of the following services: nursing services, therapeutic services, social work services, homemaker services, home health aide services, or other related services.

  • Home Health Care means the continual care and treatment of an individual if:

  • Medical cannabis pharmacy means the same as that term is defined in Section 26-61a-102.

  • Allied Health Professional means a person registered as an allied health professional with the Health Professions Council;

  • Licensed health care practitioner means a physician, as defined in Section 1861(r)(1) of the Social Security Act, a registered professional nurse, licensed social worker or other individual who meets requirements prescribed by the Secretary of the Treasury.

  • Behavioral health administration or "BHA" means

  • Behavioral health treatment means counseling and treatment programs, including applied behavior analysis, that are:

  • Department of Health and Human Services means the Department of Health and Human Services

  • Mental Health Worker means an individual that assists in planning, developing and evaluating mental health services for Clients; provides liaison between Clients and service providers; and has obtained a Bachelor's degree in a behavioral science field such as psychology, counseling, or social work, or has two years of experience providing client related services to Clients experiencing mental health, drug abuse or alcohol disorders. Education in a behavioral science field such as psychology, counseling, or social work may be substituted for up to one year of the experience requirement.

  • Rural health clinic means a rural health clinic as defined under section 1861 of part C of title XVIII of the social security act, chapter 531, 49 Stat. 620, 42 U.S.C. 1395x, and certified to participate in medicaid and medicare.

  • Licensed health care professional means a person who possesses a professional medical license that is valid in Oregon. Examples include, but are not limited to, a registered nurse (RN), nurse practitioner (NP), licensed practical nurse (LPN), medical doctor (MD), osteopathic physician (DO), respiratory therapist (RT), physical therapist (PT), physician assistant (PA), or occupational therapist (OT).

  • Behavioral health provider means a person licensed under 34 chapter 18.57, 18.57A, 18.71, 18.71A, 18.83, 18.205, 18.225, or 18.79

  • AHPRA means The Australian Health Practitioner Regulation Agency. AHPRA is responsible for regulating Australia’s registered health practitioners.

  • Licensed health care provider means a physician, physician assistant, chiropractor, advanced registered nurse practitioner, nurse, physical therapist, or athletic trainer licensed by a board.

  • Indian Health Care Provider means a health care program operated by the Indian Health Service (IHS) or by an Indian Tribe, Tribal Organization, or Urban Indian Organization (otherwise known as an I/T/U) as those terms are defined in § 4 of the Indian Health Care Improvement Act (25 USC § 1603). Indian Health Care Provider includes a 638 Facility and provision of Indian Health Service Contract Health Services (IHS CHS).

  • School health services means health services provided by a qualified school nurse or other qualified person that are designed to enable a child with a disability to receive FAPE as described in the child’s IEP.

  • Medical cannabis means the same as that term is defined in Section 26-61a-102.

  • Group health plan means an employee welfare benefit plan as defined in section 3(1) of subtitle A of title I of the employee retirement income security act of 1974, Public Law 93-406, 29 USC 1002, to the extent that the plan provides medical care, including items and services paid for as medical care to employees or their dependents as defined under the terms of the plan directly or through insurance, reimbursement, or otherwise.

  • Drug therapy management means the review of a drug therapy regimen of a patient by one or more pharmacists for the purpose of evaluating and rendering advice to one or more practitioners regarding adjustment of the regimen.
