Home Health Care definition

Home Health Care means the continual care and treatment of an individual if:

Home Health Care means medical and non-medical services, provided in your residence due to Injury or Sickness, including: visiting nurse services; physical, respiratory, occupational or speech therapy; nutritional counseling; and home health aide services. Home Health Care services must be: (1) prescribed by and provided under the supervision of a Physician; and (2) rendered by a licensed home health care provider who is not a member of your immediate family. Home Health Care does not include: homemaker, companion and home delivered meals services; nor informal care services provided by your family members.

• Note: The limitations on Therapy Services contained in this Therapy Services provision do not apply to any Therapy Services that are received under the Home Health Care provision or to services provided while a Member is confined in a Facility or to therapy services received under the Diagnosis and Treatment of Autism or Other Developmental Disabilities provision..

• The Physician shall state that confinement to a Hospital or Skilled Nursing Facility would be Medically Necessary for the treatment of the Covered Person’s Injury or Illness if the Home Health Care Plan is not provided.

• Shall be defined as a program of Home Health Care established and approved in writing by the Covered Person’s Physician for the provision of Home Health Care Services.

• Home Health Care, provided by allied health care professionals, is covered at 100%.

Home Health Care means necessary medical and medically related services provided in the patient’s home environment. These services may include, but are not limited to, nursing care, medication administration, personal hygiene, or assistance with mobility and transportation.

Home Health Care means limited part-time or intermittent skilled nursing care and home health aide services, physical therapy, occupational therapy, speech-language therapy, medical social services, durable medical equipment (such as wheelchairs, Hospital beds, oxygen, and walkers), medical supplies, and other services.

Home Health Care means medically necessary medical and medically related services provided in the injured worker’s home environment. These services might include, but are not limited to, nursing care, medication administration, personal hygiene, or assistance with mobility and transportation.

Home Health Care means certain services and supplies required for treatment of an illness or injury in the covered person’s home as part of a formal treatment plan certified by the attending physician and approved by the Plan Administrator.
