Welcome Packet definition

Welcome Packet means the materials required to be provided to new Members as set forth in OAR 410-141- 3585.
Welcome Packet. The CONTRACTOR issues welcome packets for PARTICIPANTS with coverage effective January 1. December 15 (on an annual basis)

  • The Welcome Packet shall include information about selecting a PMP, completing a health needs screening and any unique features of the Contractor.

  • Catalog means the available list of tangible personal property or services, in the most current listing, regardless of date, during the life of the contract, that takes the form of a catalog, price list, schedule, shelf price or other form that:
