Sick Leave Certificates. If the Employer requires a sick leave certificate in accordance with past practice or the Collective Agreement and the doctor charges the employee for such certificate outside OHIP, the Employer will pay for the certificate. In the alternative, the Employer may require an employee to attend an independent physician other than the employee’s own physician to provide a sick leave certificate. In such circumstances the Employer shall pay for any medical fees charged beyond OHIP in relation thereof.

Those converted cash-out credits shall then be frozen and shall be retained for pay-out when the employee resigns or retires. The balance of the current sick leave bank, and the sick leave banks of those employees who are not eligible for cash-out, shall be retained for use by the employee to a maximum bank of seven hundred and fifty (750) hours. Notwithstanding the above, an employee may use the frozen bank to top up Workplace Safety and Insurance Board payment or to pay for any unpaid sick days the extent of the credits available the frozen bank. credits!! so used cannot be replaced.

Where there is a dispute between the employee and the Insurance provider regarding the employee's entitlement to benefit coverage or payment, the Employer s I: Meet with the employee, with a Union xxxxxxx if desires, to review the issue and receive pertinent information about the dispute; Investigatethe matter with the Insurance provider in order to establish the facts related to the dispute; Where the facts indicate the employee's entitlement to benefit coverage or that payment is justified, relative to the terms of the contract with the Insurance provider, the Employer shall advocate on the employee's behalf to ensure that the terms of the contract are properly applied; Meet with the employee to review the outcome of the investigationand the rationale for the decision made. Related to Sick Leave Certificates

  • Sick Leave Accrual Full-time employees will accrue eight (8) hours of sick leave in a calendar month. Part- time employees will accrue sick leave credit on the same proportional basis that their employment schedule bears to a full-time schedule, up to a maximum of eight (8) hours in a calendar month.

  • Sick Leave Pool is intended to provide security after all other insurance benefits have been exhausted, by allowing employees to buy insurance for extended illness, or other disability. When "buying" pool days, employees convert earned days on a one (1) to three (3) basis. Similar to purchasing insurance, the employee may pick a given number of days to exchange for coverage in case of extended disability. Pool days are available for use only under the conditions listed below, and are not eligible for payout at retirement or termination.

  • Leave Accrual After the first 30 days of active military service in any one calendar year, employees shall not accrue City-paid vacation, holiday, or sick leave benefits or other forms of paid leave; provided, however, that any public employee on military leave for intermittent training periods shall continue to accrue the same vacation, sick, and holiday leave up to a maximum period of 180 cumulative days per calendar year as if the employee had not been on military leave.
