ACCESS TO UNIVERSITY HOUSING DINING. A University ID Card is required for access to University Housing dining venues. Convenience entries (entry into a dining center without an ID Card) are limited to eight per term but is not allowed for students using Duck Bucks or the Xxxxxx meal plan (UO student ID card must be present). Residents using convenience entries must show picture identification and provide their student identification number. Residents who loan their UO ID Cards to a third party will be assessed a charge in addition to the meal points or Duck Bucks used during the unauthorized entry.

ACCESS TO UNIVERSITY HOUSING DINING. A UO ID Card is required for access to University Housing dining venues. To use a convenience entry, Resident is required to show a form of picture identification and provide Resident’s student identification number. Meals may not be shared or transferred. Residents who lend their UO ID Cards will be assessed a $25 charge in addition to the meal points or Duck Bucks used during the unauthorized entry and may be subject to further disciplinary action. Residents are required to notify University Housing immediately upon the issuance of a UO ID Card that bears a new student identification number.

  • Accreditation of Online Schools The District will implement a system of accrediting its online schools, as defined in section 22-30.7- 102(9.5), C.R.S. This system shall adhere to section 00-00-000, C.R.S., including a review of the online school’s alignment to the quality standards outlined in section 22-30.7-105(3)(b), C.R.S., and compliance with statutory or regulatory requirements, in accordance with section 22-30.7-103(3)(m), C.R.S.

  • Federally Qualified Health Centers (A) The Contractor shall not restrict an Enrollee’s right to obtain FQHC services outside the PMHP through the Fee For Service Medicaid program.

  • Access to PHI Business Associate shall provide access to PHI in a Designated Record Set to Covered Entity or as directed by Covered Entity to an Individual to meet the requirements under 45 CFR § 164.524. Business Associate shall provide such access in the time and manner reasonably designated by Covered Entity. Within three (3) business days, Business Associate shall forward to Covered Entity for handling any request for access to PHI that Business Associate directly receives from an Individual.

  • University Any notice may be served upon the University by delivering it, in writing, to the University at the address set forth on the last page of this Agreement, by depositing it in a United States Postal Service deposit box with the postage fully prepaid and with the notice addressed to the University at the aforementioned address, or by sending a facsimile of it to the University facsimile number set forth on the last page of this Agreement.

  • Public Access to Nonprofit Records and Meetings If Contractor receives a cumulative total per year of at least $250,000 in City funds or City-administered funds and is a non-profit organization as defined in Chapter 12L of the San Francisco Administrative Code, Contractor must comply with the City’s Public Access to Nonprofit Records and Meetings requirements, as set forth in Chapter 12L of the San Francisco Administrative Code, including the remedies provided therein.
